Hmm the worst ideas I have for scat, besides the basic used diaper in the fac e, and one that can be used for reality (so unless we put her through a nightmare, this is probably the worst thing that could happen) is a heavily used diaper(s) to the point of bad day and torture in a diaper pail, basically someone is forced into a pail of used diapers and it's all over them. If you dont like that then I can still work with what we have, I wouldn't want you to not like what we're doing.

Well that is a good idea but I have my own ideas too, combining a bit with yours, how does this sound:
Azula being one of the highest personal member to help the Phoenix King, who is stuck in solitary confinement, and one who has made a lot of enemies, with the new fire lord Zuko's permission, she is given less rights as a citizen and is often to be punished and humiliated by others for her pasts. Her nannies could be the ones to watch over her but they aren't out of favor of getting a lil bit of revenge too. While treating her like a baby and humiliating her, Azula grows weaker and weaker and more powerless as time goes on.
