Only if they eat it in excessive quantities. Most of Pinkie's customers eat other things besides Pinkie's cupcakes, so they're fine. Pinkie and Scootaloo eat much more pony meat than their customers, so they're the ones visibly affected.

I'm not really a fan of AF characters.

Going back to the grimdark idea, we could do a story based on askpinkamenadianepie and asksadisticscootaloo. Following the plot of the blog, Pinkie comes to cull Scootaloo's parents only to discover that Scootaloo (in this universe, a sociopath with anger issues) already killed them. Pinkie decides to take Scootaloo under her wing and train her.

Consumption of pony meat en mass causes untold digestive problems for the vegetarian ponies, which is why Pinkie wears a diaper. (She lies to her friends and says its an incurable medical condition.) Scootaloo loses control too once she starts eating, putting them both in diapers.

Or how about before the Cakes have their babies, Pinkie wants to train to take care of babies. That's where Fluttershy comes in.
