You get one from each world, though the ice cream keyblade and the Ultima keyblade are pretty much mini-quests; Ice cream is to get all the ice cream ingrediants from the worlds and bring them to the ice cream shop in disney town, while Ultima is by completing the Mirage Arena, no small feat.

It's there on normal, proud, and critical modes. I promise you, you will scream at least once in frustration; Spirit and Figure make Sephiroth look like Darkside at the beginning of KH1.
