Ooh yes I like that idea. I was thinking the boys are treated as young children (wearing pull-ups, having to ask for help to use the potty, playing with toys, etc) and if they fail the exam they are treated even younger

Not sure really. But it would be fun if we could build up the test as something big and important, and make it as humiliating as possible for him. Maybe using a dildo to massage his prostate to get him hard for the examination?
The girl thing sounds good as well. Are the boys already babied, or are they forced back into diapers and stuff after they fail?

Im not quite certain what the story would be but to answer your earlier question yes there would be a physical examination at the age of 18
