Assuming no changes to your character stats, I am writing this assuming accidents are frequent for you.
On July 15th, (Monday,) you were working at the docks. However, at noon when you were stopping for lunch, you blacked out for several minutes. Upon waking up, you were wearing a soiled diaper, suckling a pacifier, and for almost twenty minutes you couldn't remove the diaper no matter how hard or what you tried.
Upon going home that evening, a small white notecard was inside an envelope eith the rest of your mail, written in blood. It said: July 19th. 5:00 PM. 1912 Warroad Lane, 64132.
Every morning since, regardless of what you wore to bed, you woke up wearing only a messy and wet diaper, suckling a pacifier, your other clothes neatly put away where they belong.

One thought: You can change your stats before we start if you want. At Continence two, you are about as potty trained as a young toddler, and probably already wear pullups anyways. If that's intentional we're okay, I just wanted to make sure you knew.

When you say 'Clydesdale...' I'm Limiting the species range to Anthro, so I assume you mean humanoid with horsey elements?
