I'm not sure if modding your game to add diapers will do too much to make them any more fun, since the change probably would only be asthetic. They do have stuff like that in second life. Also, if you're good enough, you theoretically can design your own stuff for the sims, at least sims 1 is easy to mod and add stuff. I'm not sure about user content for sims 3 though.

There is also Fox tales times, I know they also provide links for user made games. some of them are ok, though are about on par with what you would expect from something designed with RPG maker... But on the plus side, all that stuff is free...

That said, if you have your heart set on modding normal games, you could always take it up as a hobby and learn to mod the games yourself. I'm sure others would appreciate it if you posted what you came up with. It's a tedious process, but it's also one that helps you learn the game inside and out. The easiest things to do are simple texture and sprite changes, so making diaper outfits fits right into that...
