I've heard stories on both sides of the fence which has generally led me to believe that it's either something you're simply born with, a genetic predisposition, or it's created at some point in early development, when you're a baby, likely due to a variable set of factors that aren't possible to predict.
It could also be argued that EVERYBODY has an adult baby fetish, it's just that for some people it's a more important aspect or a more relevant aspect of their lives than it is for other people. Being an adult baby can manifest itself in any number of different ways, from simply enjoying children's cartoons, to coloring, to sucking your thumb, to even sucking on the end of yer pencil.
It could be argued that for most people that need to "be little", "cared for", etc is not very strong, possibly because they had a lot of love and attention as a baby/child...or they can remember it. If you had a lot of love and attention but can't recall any of it, then it's effectively the same as if it never happened.
Other factors could also influence its importancy to you. For example when I was in kindergarten there were these two girls in the class that always wanted to play house and always wanted me to be the baby and those memories/feelings of being taken care of by females that I felt an attraction to were basically permanently etched into my psyche.
