You see underneath the boy's jean shorts, he was wearing a Girls Goodnite Pull-up and had a package of them in his backpack, and he liked wearing them too, and even enjoyed acting like a baby from time to time but no one else wanted him to like those things so he ran away and fell into the Underground.
