Sweetie Belle wasn't paying Rarity all that much attention, she was more concerned that her sister was probably going to admonish her for behaving in such a way, but she sees Rarity enjoying herself as much as she is. Which gives her the courage to keep going and enjoying the dessert without any worry at all. After a few more delicious chocolate bite Sweetie Belle was quite full, definitely managing to fill herself up.
"Mmhm!" Sweetie Belle exclaims with a noise of agreement. Though as Rarity continues she realizes her sister is trying to ask about her diaper. Wiggling her rear a little, her diaper doesn't feel wet. Sweetie Belle had peed a little when that other mare had come over and been arguing with her sister, not even noticing it at the time because she was so distracted, and because her diaper did an excellent job of absorbing the urine that from where Sweetie was sitting her diaper felt completely dry. "I'm good Rarity." Sweetie Belle says, she refrains from outright mentioning that her diaper feels dry with the waitress standing right by them. Sweetie Belle wasn't too self conscious about her diapers around strangers, but that didn't mean she was going to talk about if she was or wasn't dry right as somepony was standing right besides them. She just hopes Rarity isn't going to come over and check her diaper.
