The only thing "going wrong" that I can immediately think of, is one of my crazy characters getting out of solitary and causing terror.. *shrug*
If we go the symptom route, would the character(s) already be at the hospital, or be taken there after it shows up?

Let's see , we could have the characters trying to escape from the hospital or have something go wrong or we could go go with dealing with the symptoms as they show up.

Well no , it's just we cant really do anything with it. You put some characters together in a setting and nothing will happen , you need a event or difficulty or something to solve to move them forward. Otherwise it stands still and nothing happens.

So, the sadistic personality is more of a subplot?
